IMMUNIZATION REQUIREMENTS
Immunizations - Required by Texas law. You will be notified when your child needs further immunizations.
EXTRA SET OF CLOTHES
In case accidents happen, please ALWAYS have a set of clothes in a zip loc bag in your child’s back pack.
H1N1 FLU INFORMATION
Updated federal guidelines for dealing with outbreaks of the flu, including H1N1 influenza, urge school officials to balance the risk of the flu in the communities with the disruption that school dismissals will cause in education and the wider community. Recommendation to parents is do not send an ill child to school (vomiting/diarrhea, fever, sever cough, communicable disease, chills). When deemed necessary, the school nurse may recommend that you take your child to the doctor. Take your child to the doctor if symptoms are severe. Fever needs to be clear for 24 hrs. before your child returns to school. More information about the flu and on H1N1 can be found at www. Flu.gov
ILLNESSES

MEDICATIONS
Please give at home when possible. When given at school, they must be in the original container and have a signed note giving permission for school personnel to administer. Long-term (10+ days) medication requires a yearly doctor's order form and changes require a new order.
EMERGENCY CARDS
Please notify the school of any changes in information on your child's emergency card - phone numbers, etc.
HEALTH PROBLEMS OR CONCERNS
Please report these to the nurse and any changes that occur during the year. Confidentiality is respected.
SPECIAL MEALS AND ALLERGY FORM
Use this form when your child has special dietary needs that are prescribed by a physician. It must be signed by a "recognized medical authority" and the dietary need must be medically necessary. This form must be completed each year and a copy submitted to the school nurse and cafeteria manager.
SCREENING PROGRAMS
Screening Programs - Vision and hearing are checked during the fall. You will be notified if your child needs further evaluation. If you have any concerns, please contact me.
